Medical Practice Shift Schedule: 10 Tips for Fair Planning

Create a medical practice shift schedule: 10 tips for fair, legally compliant and flexible planning in day-to-day practice. This is how shift planning succeeds.

What makes a fair practice shift schedule

A fair practice shift schedule does not happen by chance. The following ten points show what matters, from the legal basis to involving the team.

1. Factor in legal requirements from the outset

Practice tip: Check once a year whether collective agreement provisions such as the TV-MFA (collective framework agreement for Medical Assistants) are stored correctly—for example, the standard working time of 38.5 hours.

2. Consider routines and seasonal fluctuations

Every practice has its own rhythm: morning blood draws, fixed consultation hours, patient numbers that fluctuate over the year, the vaccination campaign in autumn. If such patterns are reflected in the schedule, the team is there when demand is highest.

3. Define planning rules properly once

Part-time contracts, overtime rules, allowances for on-call duty or statutory health insurance emergency services, different qualifications: If you store these rules once per employee, you do not have to reconsider them with every schedule. The effort is in the initial setup. After that, planning becomes noticeably easier. Qualifications are particularly worth the effort, because certain duties may only be staffed with the appropriate authorization. Once it is stored who is allowed to cover which on-call service, the manual check for every new shift is no longer necessary.

4. Consolidate multiple sites into one plan

Practices with multiple sites or an MVZ structure benefit from a shared shift planning overview. Employees can be scheduled across sites, and anyone stepping in at short notice does not have to be based at the same site. How such absences can be organized depends heavily on the size of the practice.

5. Capture employees’ shift preferences

6. Actively involve employees

Annual leave requests, shift swap, a spontaneous day off: When employees can help shape their shift schedule, it reduces coordination effort in day-to-day work. A schedule you were involved in is also questioned less often.

7. Build acceptance through fairness and transparency

A shift schedule is supported when it is perceived as fair. This requires understandable criteria rather than gut feeling. If you can explain why a shift is allocated the way it is, you have to defend it less often.

8. Cover short-notice absences quickly

Sick leave is part of everyday practice. What matters is quickly finding someone with the right qualifications while keeping the rest of the schedule intact. The clearer it is in advance who can step in, the fewer phone calls an absence requires. A fixed group of cover is helpful—for example, a small float pool or colleagues who voluntarily take on additional duties. This makes it possible to define in advance who planning should contact first.

9. Identify bottlenecks early

A clear shift schedule shows gaps before they become a problem. If you can already see thin staffing three weeks from now, you can reschedule calmly instead of improvising on the morning itself.

10. Keep operational goals in mind

A well-made schedule affects the numbers: less overtime, no unnecessary staffing buffer, no overqualified staff in the wrong place. Lower turnover and fewer sick leave reports are often added benefits. This saves costs and time. And a team that accepts its schedule takes care of patients more calmly.

Häufige Fragen

What legal requirements must a shift schedule in a medical practice meet?

The key basis is the Arbeitszeitgesetz: minimum rest periods, compensation for Sunday and public holiday work, maximum working time. In addition, there are maternity leave, youth employment protection, minimum wage, and the Federal Vacation Act. For Medical Assistants, the TV-MFA also regulates additional points such as standard working time.

How do you make a practice shift schedule fair?

A schedule feels fair when it follows understandable rules and takes employees’ shift preferences into account as far as possible. Transparency about the criteria is crucial. This makes it easier to explain the outcome in individual cases.

How do I take employee preferences into account in the shift schedule?

Collect your employees’ availability as well as annual leave requests and preferred days off in a structured way before you start creating the next shift schedule. The earlier the preferences are available, the easier it is to align them with legal and collective agreement requirements.
